These Terms of Service ("Terms") govern your use of wellnesscenterrxonline.com and the Wellness Center Rx weight loss program offered by the Wellness Center of Greater Cleveland ("we," "us," or "our"). By using our website or enrolling in the program, you agree to these Terms.

Eligibility

You must be at least 18 years old and located in a state where we offer care. You agree to provide accurate, complete, and current information, including your medical history.

Medical services

Medical evaluations are performed by independent licensed clinicians through our telehealth partner. Completing the eligibility quiz or paying for a program does not guarantee that you will be prescribed medication. Your clinician decides, using their professional judgment, whether the medication you selected is appropriate for you and at what dose. Individual results vary.

Our program is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.

Programs and pricing

• Program prices include your clinician evaluation, medication if prescribed, and standard shipping. Prices are shown on our Pricing page and may change with notice.
• New monthly members receive $50 off their first month. The discount applies once per patient.
• You choose your medication at checkout, and that choice is sent to your clinician for review. It cannot be changed during or after your visit. If your clinician does not approve it, the Refunds section below applies.

Billing and renewals

• Monthly plans renew automatically every month until cancelled.
• 3-month supply plans renew automatically every three months until cancelled.
• By enrolling, you authorize us to charge your payment method at the start of each billing period.

Cancellations

You may cancel at any time by contacting us at info@wellnesscenterrxonline.com or (440) 517-4877. Cancellation takes effect at the end of your current billing period, and you will not be charged again. Charges for a billing period that has already started are not refundable once your medication has been prescribed.

Refunds

• Not approved: If your clinician determines that treatment is not appropriate for you, we will refund your payment minus a $50 consultation fee, which covers the clinician's evaluation.
• Before your visit: If you cancel before completing your clinician visit, we will refund your payment in full.
• After shipment: Prescription medications cannot be returned or refunded once they have shipped, as required by pharmacy regulations.

Approved refunds are issued to your original payment method, usually within 5 to 10 business days.

Prescriptions and shipping

Prescriptions are filled by licensed U.S. pharmacies and shipped to the address you provide. You are responsible for providing a correct shipping address and for storing your medication as directed. Contact us right away if your package arrives damaged.

Your responsibilities

• Use your medication only as prescribed and only for yourself.
• Tell your clinician about any side effects, health changes, or new medications.
• Keep your account information confidential.

Limitation of liability

These Terms are governed by the laws of the State of Ohio, without regard to its conflict of law rules.

Changes to these Terms

We may update these Terms from time to time. Continued use of our website or program after changes are posted means you accept the updated Terms.
