Wellness Center Rx is the online weight loss program of the Wellness Center of Greater Cleveland ("we," "us," or "our"). This Privacy Policy explains how we collect, use, share, and protect information when you visit wellnesscenterrxonline.com or use our program.

Health information you provide for your medical care is protected health information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA). Our separate Notice of Privacy Practices describes how PHI is used and disclosed, and your rights regarding it. Information we collect

• Contact information, such as your name, email address, phone number, state, and shipping address.
• Eligibility quiz answers, such as your age range, height, weight, and health history. Quiz answers are used to show you whether you may qualify and are not added to our marketing systems.
• Medical information you provide during your clinician intake and visit, including your medical history, medications, and treatment details.
• Payment information, which is processed directly by our payment processor. We do not store your full card number.
• Website usage information, such as pages visited, device and browser type, and referring links, collected through cookies and similar technologies.

How we use information

• To determine eligibility and provide telehealth care through licensed clinicians
• To process payments, fill prescriptions, and ship medication
• To send appointment, order, shipping, and refill updates by email or text message
• To respond to your questions and provide customer support
• To improve our website and services
• To comply with legal and regulatory requirements

How we share information

We do not sell your personal information. We share information only as needed to provide your care:

• Clinicians and telehealth partners who evaluate you and, if appropriate, prescribe treatment
• Licensed pharmacies that fill and ship your prescription
• Service providers that support our operations, such as payment processing, scheduling, and secure messaging. Providers that handle PHI are bound by Business Associate Agreements.
• As required by law, such as in response to a valid legal request

Text messages and email

If you opt in to text messages, we will send care-related updates such as appointment reminders, shipping notices, and refill reminders. Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help. We keep medical details out of standard email and text messages.

Cookies and advertising

We use cookies and similar technologies to operate our website and understand how it is used. We may use advertising tools to measure the effectiveness of our ads. We do not share health information or quiz answers with advertising platforms. You can control cookies through your browser settings.

How we protect information

We use administrative, technical, and physical safeguards designed to protect your information, including encryption in transit and HIPAA-compliant systems for health information. No system is completely secure, but we work to protect your information and limit access to those who need it.

Your choices and rights

• You may request access to, or correction of, your personal information.
• You may opt out of marketing emails using the unsubscribe link, and out of texts by replying STOP.

Children

Our program is only for adults 18 and older. We do not knowingly collect information from children.
