Telehealth Consent
What to know about receiving care through telehealth, including its benefits and limitations.
Last updated October 20, 2026
Telehealth is the delivery of healthcare services using secure electronic communication, such as video visits, online questionnaires, and secure messaging, when you and your clinician are not in the same location. By using the Wellness Center Rx program, you consent to receive care through telehealth as described below.
How telehealth works in our program
- You complete an online medical intake about your health history, medications, and goals.
- A licensed clinician reviews your information. Depending on your state's requirements, this may include a video visit or a review of your written answers.
- If treatment is appropriate, your clinician may prescribe medication, which a licensed pharmacy ships to you.
- Follow-up care and dose adjustments may also take place through telehealth.
Benefits
- Access to care from home, without travel or waiting rooms
- More flexible scheduling and faster follow-up
- Ongoing support from your care team between visits
Risks and limitations
- Your clinician cannot perform a physical exam. Some conditions may need an in-person evaluation, lab work, or a referral.
- Information may be incomplete or delayed due to technical problems, such as poor internet connection or equipment failure.
- Although we use secure systems, there is a small risk that electronic information could be accessed without authorization.
- Your clinician may decide that telehealth is not appropriate for you and recommend in-person care instead.
Your responsibilities
- Provide complete and accurate information about your health history and medications.
- Follow your clinician's instructions and report side effects or health changes promptly.
- Seek in-person or emergency care when needed. Telehealth is not for emergencies; call 911.
Your rights
- You may decline or stop telehealth care at any time without affecting your right to future care.
- Your health information is protected under HIPAA and applicable state law.
- You may request copies of your medical records.
- You may ask questions about your care at any time.
Consent
By checking the consent box when you enroll, you confirm that:
- You have read and understand this information.
- You agree to receive care through telehealth.
- You understand that a licensed clinician will decide whether treatment is appropriate, and that no specific outcome is guaranteed.
